I answered "Ladies do not come forward until lead" because this is the best answer that you made available. I do think that the default is that the follower should start forward on the 1, but the leader can decide to lead a rock-step instead and the follower needs to be ready to respond to that.
"Hollywood" I don't know Hollywod style, but there is a Hollywood style teacher here and most of his students seem to default to a rock step on the 1,2.
"offshoot" Ladies may come forward in any form of swing dancing, depending on what the leader leads. (I also think that "offshoot" might be somewhat offensive to those people who do variations other than Savoy or Hollywood. I consider myself to be Savoy style, but my first Lindy teacher came to lindy from West Coast, and I did most of my social dancing in the San Francisco area and so have a lot of the SF-Groovy style.)
"twist twist" I think that the Followers Twist requires at least a little bit of forward momentum, and can be a walk forward depending on what the leader leads. No lead and the twist is in place. Stong lead forward and the twist is a movement.
As with everything else, though, your personal experience will decide how you interpret the 1,2.
